Abstract

Currently South Tangerang Public Health Service in the reporting system of monitoring and evaluation activities are still manually using the application spreedsheet so often encountered obstacles both in the process of inputting, editing, deletion of data as well as data transmission. It therefore requires an integrated computing system so that these constraints can be minimized and the system can also be accessed by both internal and external Health Department. In this research created a system that was designed with the waterfall method and the programming language used is PHP Script whereas for data storage media with MySQL. The results obtained from this study showed that the system is made to minimize the constraints on earlier systems can also be accessed easily for such a system based websites.
